Filling out a Search Request Slip
Once you have verified that the book is, in fact, among the missing, retrieve a 'Search Request' form for the patron to fill out. You may assist the patron, if time permits, and provide the item information required for the form. Once the form is filled out, you must verify all the information.
- Is the Call Number correct?
- Does it match the author and title?
- 'Author' and 'Title' should be legible and complete. The full name of the author and the full title (with sub-title) is required.
- Patron information must include the patron's full name and the patron's barcode or ID number.
- A brief address is required, in case there is some confusion with the patron's other ID supplied.
- Verify that there is a current phone number supplied, for future contact.
- Have the person supply a date whereby the Item would no longer be of use to him.
- Search staff will notify the patron, if the book is found within the specified time frame. If found after that date, the Searcher knows the book can be released into regular circulation.
- All information must be legible.
When all the information has been supplied and verified, date and sign your name on the reverse of the slip. This gives the Searcher a point of reference as to when the Search was initiated and a staff member to confer with, if there are any questions that arise.
Place the Search Request slip in the designated area at the Circulation Desk. The Searcher will retrieve the slips throughout the day or the following morning.
